Recertification Templates and Forms Start your recertification here. This packet serves as a guide for determining which activities count for archival recertification credits, the number of credits an activity is worth, and the maximum number of credits you can claim in each section. More details on earning credits can be found on the Recertification Definitions and Special Instructions page. Petitioners should use this packet to help assemble their detailed Qualifying Archival Experience Document and as a worksheet for adding up earned credits before transferring them to the online form. Note: If you prefer to submit a paper petition, you will use the forms in this packet in place of the Recertification by Petition Online Form listed below.
We suggest that you download and use this Word template when formatting the detailed qualifying archival experience portion of your petition. Note that this template corresponds to pages Pet-6 through Pet-9 of the Certification Maintenance Packet.
Once you have compiled your archival recertfication credits and completed your detailed Qualifying Archival Experience Document, you can save some postage and trees by submitting your petition via this online form. Once you start filling out the form, you will not be able to save it, so it is important that you have completed the worksheet and template ahead of time. The form has a field into which you can upload your Qualifying Archival Experience Document (as a Word or PDF file) and submit it electronically. Note that this form corresponds to pages Pet-1 through Pet -5 of the Certification Maintenance Packet.
